CGW Structures Forms

All application forms for our services are available on this page. Follow these four easy steps to fill out and send your application forms to us:

Step 1
Download the form you require from the list below.
Step 2
Enter your information into the form.
Step 3
Save a copy of your form using the "File -> Save as..." command at the top of your screen.
Step 4
Print and sign the form.
Step 5
Fax the form with any attachments to CGW Structures on (07) 3231 8955 .

You can continue as you do now, by establishing the company with a CGW team member as the first officeholder and shareholder of the company.

We will then appoint your nominated officeholders and transfer the shares to your nominated shareholders.

You should be aware that choosing this option may cause some delay in the ASIC record being updated to show your nominees as officeholders and shareholders of the company. While we do everything we can to expedite the process, we sometimes experience ASIC processing delays which are out of our control.
If you would prefer your nominees to be the first officeholders and shareholders of the company and not have a CGW team member involved, we can now do this.

If you choose this option you will need to send completed and signed consents to act as director, secretary and/or public officer and shareholder from your nominees with your application in order for it to be processed. You can download the consents here.

No matter what option you choose, you can either:

download the forms and consents (if you choose option two above), complete and sign and fax, email or post the forms to us; or
complete the application online and submit it to us through our website.
